The 26-year-old has been at Rennes since 2020, where he made 66 appearances in Ligue 1 over two seasons as a first-team regular, scoring five goals as a centre-back.
His performances have caught the eye in the Premier League and West Ham are ready to bank on the Moroccan international.
The Evening Standard reports that Aguerd could be revealed as a hammer by the end of the week after traveling to London for his medical treatment and the deal will be an initial fee of at least £20million which could rise significantly with add-ons.
The left-footed centre-back will solve that particular problem West Ham have had during this transfer window and manager David Moyes is keen to fill that gap in his squad.
The report also says the Hammers are ready to listen to offers for Issa Diop, who has fallen dramatically out of favor over the past two seasons.
The 25-year-old had been linked with a big-money move to Manchester United after an impressive first two seasons at West Ham but has since struggled to maintain a place in the first-team, making just 10 starts in the last season Premier League completed.
